William Osler Health System offers inpatient rehabilitation at the Brampton Civic Hospital site.
The Inpatient Rehabilitation programs are progressive rehab programs designed for adults who are able to tolerate a high-intensity rehab program to re-build their strength and independence, and with medical and nursing care needs that cannot yet be met in the community.
The Functional Enhancement Service (FES) is a lower intensity rehabilitation program located within our Complex Continuing Care unit. FES is designed for people who have longer-term illnesses or disabilities, limited activity tolerance, and may need more medical/nursing care.
Most patients are referred by the acute inpatient units at Osler (Etobicoke General Hospital or Brampton Civic Hospital), and external applications will be considered if referred by a physician or another acute care facility.
Headwaters Health Care Centre (HHCC), located in Orangeville, provides general inpatient rehabilitation to assist patients to achieve their maximum function prior to returning home. Our rehabilitation beds are situated on our Complex Continuing Care unit and general rehabilitation is provided for patients coming from acute inpatient units at HHCC.

Located at Brampton Civic Hospital (patients under 70 years of age) & Peel Memorial Centre for Integrated Health & Wellness (patients over 70 years of age)
Community Outreach Stroke Rehab (COSR) Program – The Central West LHIN Home and Community Care team in collaboration with the Outpatient Rehab Program at William Osler Health System (Osler) offers a program for patients who have a mild stroke discharged from acute care. A Rehabilitation Care Coordinator will complete a comprehensive assessment in your home and will serve as your point person in your rehab journey. Regulated health professionals such as physiotherapists, occupational therapists and speech language pathologists see patients in the outpatient rehab department at Osler. A patient centered treatment plan is developed and treatment location occurs in both the outpatient program and in your home. The goal is to enable patients to return to previous level of functioning and re-integrate into the community. A referral from a physician or Nurse Practitioner is required for this program.

All Neurological Rehabilitation Services at Osler require a referral from a physician, and completion of referral form
Outpatient Physiotherapy provides assessment and treatment of patients recovering from total joint replacements and acute musculoskeletal conditions. Programs offered at Etobicoke General Hospital & Brampton Civic Hospital. Referral by a physician is required.
Outpatient Occupational Therapy (Hand & Upper Extremity Program) offers post-surgical assessment, treatment, and splinting for post-surgical hand and upper extremity procedures, and acute hand injuries. Programs offered at Etobicoke General Hospital & Brampton Civic Hospital. Referral by a physician is required.

Headwaters Health Care Centre does not offer outpatient rehabilitation. Outpatient physiotherapy services are available at public and privately funded clinics Orangeville and surrounding communities.
COMMUNITY BASED REHABILITATION (HOME AND COMMUNITY CARE)
Home and Community Care of the CW LHIN offers the following:
Exercise and Falls Prevention Program (Seniors) : Funded by the Ministry of Health and Long-Term Care, the Central West LHIN offers seniors group exercise and falls prevention classes in over 50 locations throughout the Central West community including retirement homes and community centres. The classes are led by a physiotherapy assistant (PTA) and are supervised by a physiotherapist (PT). The goal of these classes are to improve and maintain functional performance through strength, balance, and mobility and to provide education to reduce the risk of falls. With your consent, the physiotherapist may consult with your family doctor regarding any exercise restrictions. Registration is required to attend the classes. Stroke Exercise Program (Seniors): This specialty group exercise and fall prevention class geared towards stroke survivors and consists of strengthening and balance exercises in a small group-based setting. These classes are led by a physiotherapy assistant under the supervision of a physiotherapist and are offered two times per week to qualifying individuals. An assessment is required in order to determine if the stroke group exercise program is best suited for your needs. COPD Exercise Program (Seniors ): This specialty group exercise and falls prevention class geared toward individuals who have a formal diagnosis of Chronic Obstructive Pulmonary Disease (COPD) and could benefit from exercising in a group-based setting. The focus is on breathing, lung capacity and improving exercise tolerance. The individual may be a graduate of a local Pulmonary Rehabilitation Program or have a recent new diagnosis of COPD. A physiotherapist conducts an assessment in order to determine if the COPD group exercise program is best suited for your needs. With your consent, the physiotherapist may consult with your family doctor regarding any exercise restrictions. Registration is required to attend the classes. In home Rehabilitation (Children, Youth, Adult, Seniors): The Central West LHIN, Home and Community Care team also provides a range of goal-oriented and time limited in home rehabilitation programs for who are unable to access programs offered in ambulatory care settings. Registered physiotherapists, occupational therapists, speech language pathologists and registered dietician or social workers from contracted service provider organizations deliver the programs. The goal is to optimize function in the home and connect patients with community supports and services. The Home Independence Program (Adults, Seniors) is an 8-week short-term, home-based restorative care program designed using a best practice approach to improving senior’s independence and prevent decline. The program is led by an Occupational Therapist (OT) who assigns activities to the personal support worker (PSW) to promote skill building and strength development through participation in activities of daily living (e.g. bathing, dressing, grooming). Patient centered goals are defined at program intake. A referral from a regulated health professional is required for this program.
School Based Rehabilitation (Children, Youth): Children and youth in publicly funded and private schools and those who are being home schooled. The goal is to assist them in pursuing their education and require the service in order to attend school, participate in school routines and receive instruction, including receiving satisfactory instruction at home. These services include physiotherapy, occupational therapy, and speech language pathology.
