INPATIENT REHABILITATION

William Osler Health System offers inpatient rehabilitation at the Brampton Civic Hospital site only. 
  • The Active Rehabilitation & Reactivation programs are progressive rehab programs designed for adults who are able to tolerate a high-intensity rehab program to re-build their strength and independence, and with medical and nursing care needs that cannot yet be met in the community. 
  • The Functional Enhancement Service (FES) is a lower intensity rehabilitation program located within our Complex Continuing Care unit.  FES is designed for people who have longer-term illnesses or disabilities, limited activity tolerance, and may need more medical/nursing care. 
   
 
Headwaters Health Care Centre (HHCC), located in Orangeville, provides general inpatient rehabilitation to assist patients to achieve their maximum function prior to returning home.  Our rehabilitation beds are situated on our Complex Continuing Care unit and general rehabilitation is provided for patients coming from acute inpatient units at HHCC. 

 
OUTPATIENT REHABILITATION

William Osler Health System offers the following outpatient rehabilitation services:
  • Outpatient Neurological Rehabilitation (Occupational Therapy, Physiotherapy, Speech-Language Pathology) – single service neuro rehab for a range of neurological conditions including stroke, acquired brain injury, and brain tumour resections.
    • Located at Brampton Civic Hospital (patients under 70 years of age) & Peel Memorial Centre for Integrated Health & Wellness (patients over 70 years of age)
 
  • Community Outreach Stroke Rehab (COSR) Program –The Central West LHIN Home and Community Care team in collaboration with the Outpatient Rehab Program at William Osler Health System (Osler) offers a program for patients who have a mild stroke (Alpha FIM >80) and are discharged from acute care.  The Rehabilitation Care Coordinator from Home and Community Care completes a comprehensive assessment in the patient’s home, assist patients to set personalized goals and serves as a point person in the patient’s rehab journey.  Patient-centered goal setting using the COPM© is a core feature of the program and drives the intervention approaches.  OT, PT and SLP assess patient in the outpatient rehab department at Osler and develop treatment plan. The patient centered interventions are provided by community-based assistants (OTA, PTA, CDA) in both the outpatient program and in the patients home and community under the direction of the outpatient therapists. The goal is to enable patients to return to previous level of functioning and re-integrate into the community.  Referrals are triaged by Osler. Please include a complete discharge summary, alpha FIM score, and documentation confirming a diagnosis of a stroke.
 
  • Outpatient Physiotherapy provides assessment and treatment of patients recovering from total joint replacements and acute musculoskeletal conditions. Programs offered at Etobicoke General Hospital & Brampton Civic Hospital.

  • Outpatient Occupational Therapy (Hand & Upper Extremity Program) offers post-surgical assessment, treatment, and splinting for post-surgical hand and upper extremity procedures, and acute hand injuries. Programs offered at Etobicoke General Hospital & Brampton Civic Hospital.
 
  • Headwaters Health Care Centre does not offer outpatient rehabilitation. Outpatient physiotherapy services are available at public and privately funded clinics Orangeville and surrounding communities. 
 
COMMUNITY BASED REHABLITATION

Home and Community Care of the CW LHIN offers the following:
  • Community Seniors Exercise and Fall Prevention classes (Seniors) are open to all seniors aged 65+ years old and do not require a referral. Registration is required to attend the classes and all participants are assessed by a physiotherapist prior to commencing a program and the classes are led by a physiotherapy assistant (PTA).  Classes are available in over 50 locations throughout the Central West LHIN in community settings such as retirement homes, community centres and faith based settings. The classes are led by a physiotherapy assistant (PTA) and are monitored by a physiotherapist (PT). The goal of these classes are to improve and maintain functional performance through strength, balance, and mobility and to provide education to reduce the risk of falls.
 
  • Stroke Exercise Program (Seniors): This specialty group exercise and falls prevention class is geared towards stroke survivors and consists of strengthening and balance exercises in a small group-based setting. Registration is required to attend the classes  and an assessment is conducted by a physiotherapist in order to determine if the stroke group exercise program is best suited for the patient’s needs.
 
  • COPD Exercise Program (Seniors): This specialty group exercise and falls prevention class is offered to individuals who have a formal diagnosis of Chronic Obstructive Pulmonary Disease (COPD) and could benefit from exercising in a group-based setting with a focus on breathing, lung capacity and improving exercise tolerance. The individual may be a graduate of a local Pulmonary Rehabilitation Program or have a recent new diagnosis of COPD. Registration is required to attend the classes  and an assessment is conducted by a physiotherapist in order to determine if the COPD group exercise program is best suited for the patient’s needs.   • In home Rehabilitation (Children, Youth, Adult, Seniors): The Central West LHIN, Home and Community Care team also provides a range of goal-oriented and time limited in home rehabilitation programs for who are unable to access programs offered in ambulatory care settings or who are home bound. Registered physiotherapists, occupational therapists, speech language pathologists and registered dietician or social workers from contracted service provider organizations deliver the programs. A referral from a health provider is not required, however is highly recommended if there is a specific course of treatment being prescribed. Patients recently been hospitalized, who have undergone an acute surgical intervention or those who are frail and elderly may be referred for in home services.   • School Based Rehabilitation (Children, Youth): Children and youth in publicly funded and private schools and those who are being home schooled.  The goal is to assist them in pursuing their education and require the service in order to attend school, participate in school routines and receive instruction, including receiving satisfactory instruction at home. These services include physiotherapy, occupational therapy, and speech language pathology.
