Description of Services: Helps hospitals and community-based programs connect people with Acquired Brain Injury (ABI) to the services they need * single point of entry to outpatient ABI rehabilitation and ABI community services such as recreational, vocational and supportive housing programs * central source of ABI information for patients and families to help them find and access health care and social services * navigation support to find and access inpatient and community-based rehabilitation and support programs * assist individuals with complex ABI needs to access services

Eligibility - Population(s) Served: To apply for ABI programs and services individuals must have a diagnosed brain injury and complete the Toronto ABI Network referral form. Toronto ABI Network members are publicly-funded providers of brain injury services and/or support in the Greater Toronto area
Application: Select and complete the appropriate form and fax directly to 416-597-7021 * To apply for inpatient rehab, complete the Inpatient Form * For outpatient rehab or community services, use the ABI Community Referral Form
