Description of Services: Outpatient Mental Health offers short term psychiatric consultation, group psychotherapy, and limited short term individual psychotherapy for patients with mental health concerns. Treats all patients with meatal health and substance use disorders

The Central Intake and Triage Clinicians screen the patient and provide a brief assessment and referral. The referral may be for a consultation, consultation and treatment, or telephone advice, depending on the patient’s needs

Application: Referrals accepted from patients and physicians. Referring psychiatrists or family physicians should complete a Centralized Intake and Triage Referral Form and fax it to 905-494-6757
