| Description of Services: |
Basic to advanced level diabetes education and ongoing care to children and families, youth and adults who are at risk or have Type 1 diabetes, pre-diabetes, Type 2 diabetes, pre-gestational diabetes, and insulin pump starts and management. Our program consists of a multidisciplinary team including Registered Nurses CDE, Registered Dietitians CDE, Social Workers and physicians Services are available for:
