| Eligibility - Population(s) Served: | Adults 18 years of age or older, have comorbid conditions associated with their diabetes, such as vascular disease, renal failure, neuropathy, impaired vision, mental illness, financial difficulty accessing care, have inadequate glycemic control and/or significant co-morbidities which impact on glycemic control; and/or, have had recurrent diabetes emergencies or have been hospitalized with poorly controlled diabetes and higher readmission rate | ||
