Description of Services: Francophones in the Toronto area living with many or complex issues can access support services through the Francophone Healthcare Navigator program

The primary role is to help create effective links between appropriate health and social resources * assist clients to engage pro-actively with community, family and with the healthcare system in order to improve their access to primary care (doctors, nurse practitioners, specialists...etc) * works with primary care providers with the goal to improve the quality of life of each individual through client centered care and by offering treatment/services in the most appropriate setting

The Francophone Navigator may assist you with determining your needs and accessing:
  • primary health care providers, chronic disease management (eg. heart disease, cancer, diabetes, stroke, arthritis and obesity)
  • addictions and mental health care
  • social services
  • health care and acute
  • care services at home
