Integrated Care Clinic
Description of Services: Staffed with qualified teams of health care professionals, Integrated Care Clinics provide a variety of specialized nursing services to patients of all ages. Depending upon the specific clinic, these services may include:

  • Wound care
  • Drain care
  • Injections
  • Central line maintenance
  • Catheter care
  • Enterostomal therapy/ostomy care and education (ET nurse available to see patients at the clinic once per week)
  • IV therapy and IV access maintenance
  • Chemotherapy disconnects
  • Lymphedema care
  • Diabetes management counselling and education
  • Education on self-management including but not limited to diabetes and injection

Fees: Covered by OHIP
Eligibility - Population(s) Served: •not open to general public •must be assessed and referred to the clinic by Ontario Health atHome Central West staff •visit to clinic by appointment only, no walk-ins accepted •patients must be covered by OHIP
