| Description of Services: |
The Clinic supports seniors at high risk of injury due to a fall. Assessments include interviews, questionnaires, standardized fall risk and frailty testing, physical examinations and cognitive screening, when appropriate Conditions treated:
Patient exclusions: patients who are wheelchair bound or use wheelchairs for their primary mode of mobility; patients for whom fall issues are primarily related to their home environment (home safety assessment more appropriate); patients who are unable to attend an outpatient clinic; patients who require assistance for all gait; transfers (refer to Reconditioning clinic); patients post-CVA who are eligible for the stroke program Patients are assessed and receive a coordinated care plan from a team of experts |
