Program: Health Care Connect Program

Description of Services: Program that refers people without a regular family health care provider to physicians and nurse practitioners who are accepting new patients in their area

  • voluntary program for both patients and participating family health care providers - patients register by calling 811, with a centrally located patient representative, or they can register online www.ontario.ca/healthcareconnect
  • patients must have a valid Ontario Health Card Number to be eligible for the program
  • once patients are registered, information is forwarded to Ontario Health atHome office that covers the area where the patient resides
  • Care Connectors are based locally in Ontario Health atHome-Brampton offices and facilitate the connection between patients and doctors/nurse practitioners
